Membership eligibility is primarily limited to Banner Health System and Sonora Quest Laboratory employees and small employee groups associated with or affiliated with Banner Health and other health care providers. Once you're a member, you are always a member! Don't worry about switching employers or moving out of state, we will always welcome you at Banner Federal and we have plenty of services to meet your needs, near or far! Plus, once you join, your immediate family members are also eligible to join.

To get started, you will need the following information to complete your application:

• Provide proof of membership eligibility (employee number, family member, etc)
• Your personal information including your driver's license or state ID number and social security number
• Information regarding your designated account beneficiary
• Personal information for your joint account owner (if applicable)
• Your existing bank's routing number and account number if you decide to fund your account today*
